Required Forms

We must receive the following for each child: 

• Registration Form (or Apply On-Line)
• Applicant Statement
• Teacher Evaluation Form(s)
  •  Nursery - Year 2 (ages 3-5) Teacher Evaluation Form
  •  Year 3 - Year 6 (ages 6-10) Teacher Evaluation Form
  •  Year 7 - Year 13 (ages 11-17) Mathematics Teacher Evaluation Form
  •  Year 7 - Year 13 (ages 11-17) English Teacher Evaluation Form
• Most recent school report / report card (where appropriate) to be sent with Evaluation Form(s) directly from current school
• Recent photograph

Your application will be processed when the admissions office receives your completed registration form, accompanying paperwork and registration fee.
