The following forms can be printed off and sent to the School Office

Bus registration 2008

 Pupil Information Sheet

This form is issued at the beginning of each school year and contains all contact and medical details for your child. It also allows the school to obtain medical attention for your child in the case of an emergency. Please send any changes to the School Office.

Change of Details Form

Please complete if any of your contact numbers or addresses change.

Medical Authorization Form

Please complete if you wish the school to administer your child's medicine.

Volunteer Form

Please complete this form if you are interested in voluntary work at the school. The completed form should be returned to the school office marked for the attention of Heather Holmes - Communications Coordinator. Further details of duties can be found on the Volunteers page.

Immtrac form            Immtrac letter

The Houston Department of Health & Human Services has implemented a voluntary scheme called the Houston-Harris County Immunization Registry. This registry will record your childrens' immunizations in a central location and send reminders of up-coming immunizations.
